What happened to Cole Ragans? Live injury updates for Guardians vs. Royals on 4/08/26

In the bottom of the first inning of today’s game between the Guardians and the Royas, Kansas City ace Cole Ragans left the game after getting two quick outs and then falling apart due to an injury he suffered when Guardians 3B Jose Ramirez lined a ball directly off Ragans’ left (pitching) hand.

Below is a video of the line drive that caused the injury.

Cole Ragans’ 2026 season has already felt precarious, and today only sharpened that. The raw line still says there is frontline stuff here: through his first two starts he had a 3.60 ERA with 13 strikeouts in 10 innings, and his second outing against Minnesota looked much more like the ace version, with six innings and eight strikeouts. But the season has not felt stable. His opener against Atlanta was messy—homers, walks, short outing—and now today’s start against Cleveland added a different kind of familiar problem: bad injury luck.
