The tournament doesn’t wait to throw haymakers, does it? Right out of the gate, we get a First Four matchup between two programs with real second-weekend potential. North Carolina, a blue blood with expectations that never fade, meets San Diego State, a team built to ruin the nights of more talented rosters. The winner heads to the South Region’s main bracket to face six-seed Ole Miss. But first, they have to get through each other. With UNC’s offensive explosiveness clashing against SDSU’s smothering, slow-it-to-a-crawl defense, this one has all the makings of a classic March stylistic battle. Felt like I was barely gone, but so happy to be back with you all. Here’s my preview, prediction, pick, and best bet for this March Madness First Four college basketball game between San Diego State and North Carolina tonight.

San Diego State vs. North Carolina preview, prediction
North Carolina’s inconsistency has been maddening, but at their best, they look like a team that should be comfortably out of the First Four conversation. Their offense can ignite or implode in equal measure, and their fate is tied to R.J. Davis, the ACC Player of the Year who’s been averaging 22.1 PPG over his last eight while hitting 45.2% from three in that stretch. His ability to create his own shot is UNC’s lifeline, but the Tar Heels need more than just his scoring. Seth Trimble has stepped into a larger role, shooting 38.9% from deep in conference play. Jae’Lyn Withers, too, provides much-needed versatility, grabbing 7.2 RPG over his last five while defending multiple positions. UNC’s rebounding edge is +5.8 in its last five games, but turnovers remain a glaring issue. They’ve coughed the ball up 14.1 times per game in that same span. Against an SDSU team that forces 15.2 turnovers per game, that’s a dangerous recipe.
San Diego State doesn’t have the offensive firepower of UNC, but their defense is pure punishment for teams that rely on rhythm. The Aztecs hold opponents to 63.6 PPG (14th nationally) and allow just 0.83 points per possession in the half-court. UNC won’t always get the clean looks they’re used to. Nick Boyd has been SDSU’s biggest offensive spark, averaging 14.3 PPG in his last four. That’s all while shooting 42.7% from three, too.
But SDSU’s real path to victory lies in its ability to turn stops into transition points. They’ve been outscoring opponents by 7.4 PPG off turnovers in their last five (including tournament play), and UNC has been prone to sloppy stretches. That’s especially true when they face aggressive pressure. The Aztecs also have an edge late—while UNC has struggled at the line at times, SDSU is knocking down 77.8% of its free throws over its last five games. That’s crucial in a tight matchup.
North Carolina at San Diego State pick, best bet
Rebounding swings games in March, and it might just decide this one. If UNC controls the boards and R.J. Davis dictates the offense, the Tar Heels will pull away. But if SDSU drags this into the mud, forces UNC into low-quality, forced possessions, and feasts on turnovers, the Heels could unravel under pressure. The -4.5 spread leans toward UNC’s talent edge, sure. But the Aztecs thrive in disruptive, grindhouse battles. That’s exactly what they’ll try to create.
The total (currently 142.5) is a battle of extremes—if UNC turns this into a track meet, the over cashes without a sweat. If SDSU’s half-court defense controls the tempo, the under becomes the sharp side. I wouldn’t touch it until the game goes live.
But the first-half spread, now: UNC -2.5 (-105) is the real play, especially with SDSU ever-so-slightly favored there. The Tar Heels have imposed their will early in five of their last six games, outscoring opponents by a combined +38 in first halves during that span. Meanwhile, SDSU’s offense tends to find its footing later—the Aztecs have averaged just 29.4 first-half points over their last five, nearly four points lower than their season average. If UNC builds an early cushion, the Aztecs could be forced into catch-up mode—not where they want to be.
More broadly, this game comes down to whether SDSU can smother UNC’s firepower before it gets rolling. If the Heels play in space and get out in transition, they’ll win and cover. If the Aztecs turn this into a slugfest and force UNC into long, grinding possessions, this one goes to the wire. Either way, whoever emerges will have passed a brutal test before even reaching Ole Miss.
Still, the feel for the first-half spread is too strong. UNC -2.5 in 20 minutes flat.
